Welcome to our office! Your initial visit to our office will include a personalized welcome from our New Patient Concierge, who will meet all your needs during the interview and treatment process. During your interview, we will discuss your goals and objectives for optimum oral health and the appearance of a “Smile of Your Dreams”.

To lower your wait time, you are welcome to complete your Patient Paperwork Forms prior to your appointment.

Our New Patient Concierge will walk you through your entire experience of “Getting to Know You”. We are committed to acquiring the following to give you a comprehensive diagnosis and recommendations:

  • All Necessary X-Rays
  • Photographs Internal & External
  • Diagnostic Models
  • Periodontal Tissue Health Assessment
  • Oral Cancer Screening
  • Tooth Decay Assessment
  • Blood Pressure Screening

These procedures and records allow our doctors to assess your level of health before presenting you their findings and recommendations that will transform your smile. We anticipate serving you with the utmost care each time we see you.

Insurance

We will be more than happy to file claims to all insurance companies as a courtesy to our patients. We will do everything we can to help you recover the most from your benefits.

Payments

Payment for treatment is due the day of your appointment. Forms of payment accepted by the office are checks, cash, or any major credit card.
